The following is a credit application to open an account with American Industrial Transmission, Inc.

To aid us in quickly processing your request, please use only trade references.
Banks, leasing companies, and other financial institutions are not able to give us the information we are looking for.
We consider trade references to be your sources for tires, batteries, engines, seats, and other repair parts.
Be certain to sign the application. This gives us your permission to ask for privileged information.
We require a sales tax exemption certificate be on file before purchases are made. Our customers are forklift dealers and equipment repair facilities. We only sell to those resellers that will complete our provided tax form or submit one of their own.
Until we have received your completed application, we will ship your order if paid by COD (cash or money order) or a credit card.
Please allow 5 days for processing of your request for open account.
